If you are applying for a Reception class place for September, you will need to complete the Local Authority common application form.
The Local Authority Reception common application form is available on www.eadmissions.org.uk.
If you are applying for Reception under certain criteria of the admissions policy, you may also need to complete the SGHS application form (called the Supplementary Information Form).
